Pre-Consultation Preparation: Food Diary

To get the most out of your consultation, please keep a three-day food diary noting everything you have had to eat or drink over the three days, including nutritional supplements.

Some useful questions to consider prior to consultation are: -

1. What are you hoping to achieve from a dietetic consultation?
2. What are your health and dietary concerns?
3. How knowledgeable are you about nutrition?
4. What recent dietary changes have you made ?
5. How motivated are you to make dietary changes?
6. What support have you got from friends, colleagues, family?
7. How are your cooking skills?
8. What follow up support do you need?

In some cases your GP may be contacted with your approval. It is helpful to bring a list of prescribed medications and recent records of blood tests if relevant.
